The word already carried a judgment: that which is foreign, bizarre, primitive. When European psychology and psychiatry began to study sexual deviations, they borrowed this definition and applied it to a different phenomenon.

In 1886, psychiatrist Richard von Krafft-Ebing published Psychopathia Sexualis, a catalogue of “perversions” that first defined fetishism as a pathology. For Krafft-Ebing, a man who became aroused by an inanimate object was ill. His classification had enormous influence on medicine and law for over a century.

Today we know that reality is more complex. Sexual fetishism is not in itself a disorder. It becomes one only when it causes clinical distress or impairs a person’s functioning. This distinction is crucial: many adults have fetishistic preferences without these interfering with their life or anyone else’s.

From the diagnostic manual to normalization

The Diagnostic and Statistical Manual of Mental Disorders (DSM) has changed its position on fetishism several times. In the early editions, any fetishistic interest was considered pathological. Starting with the DSM-III (1980), the diagnosis required that the behavior caused “clinically significant distress” or impaired “important areas of functioning”.

The DSM-5 (2013) introduced a further distinction: fetishism is a disorder only if the person feels distress about their interest or if it harms others. It is not enough for a fetish to be present. Stricter criteria are required, such as a duration of at least six months and the intensity of arousal.

This evolution reflects a broader cultural shift. Contemporary research tends to view fetishistic preferences as variations of the human sexual spectrum, not as anomalies. The World Health Organization, with the ICD-11, has followed a similar path, eliminating the word “perversion” and instead speaking of “paraphilias” only when they cause harm or distress.

It is not uncommon to hear someone ask: but is a fetish always tied to an object? The short answer is no. The clinical term also includes situations in which arousal is tied to non-genital body parts (feet, hair) or to specific materials (rubber, leather, latex). Those who practice it often say that the object or body part acts as a catalyst for an amplified sensory experience.

Differences from other practices and cultural boundaries

Fetishism is sometimes confused with role play or with BDSM. The difference is subtle but real. In role play, arousal arises from playing a role together with a partner. In BDSM, from the power dynamic and explicit consent. In fetishism, the object or body part is the center of erotic attention: it can be used alone or with others, but it does not necessarily require a partner.

Some cultures have historically integrated fetishistic elements into religious or social rituals. In Japan, the practice of kinbaku combines bondage and aesthetic admiration of ropes. In some African societies, objects such as masks or fabrics had a spiritual meaning before being redefined as fetishes by colonial literature.

The medicalization of fetishism has had a side effect: it has obscured the difference between a harmless preference and a problematic behavior. For example, an interest in feet is not clinically relevant if the person lives peacefully with their sexuality. It becomes a disorder only when the fetish replaces every other form of intimacy or causes anguish.

Those who recognize themselves in a fetishistic interest today have access to resources that did not exist fifty years ago. Online communities, support groups, and trained professionals make it possible to explore these aspects without shame. Modern sex education emphasizes that the line between healthy and pathological does not run through the specific object, but through the context and the person’s lived experience.

Safety and consent

Even the most harmless fetish requires attention to safety and consent. When the interest involves a partner, communication is essential. Speaking openly about what you desire avoids misunderstandings and allows you to negotiate limits. There is no fetish that justifies the absence of a safe word or of a clear agreement.

Some material fetishes involve specific physical risks. Latex or rubber items can cause allergies or breathing difficulties. Rigid or sharp objects require precautions to avoid injury. The breathability of materials is also a factor to consider during prolonged sessions.

Mental health should not be neglected. If a fetish becomes the only way to experience pleasure, or if the person feels intense shame, talking to a therapist specializing in sexology can help. Not to “cure” the fetish, but to integrate it into a balanced and satisfying sex life.

Sources and references

Sources:

Wikipedia: Sexual fetishism – historical and clinical overview

Wikipedia: Sexual fetishism – DSM criteria and history

American Psychiatric Association – DSM-5 criteria for fetishistic disorder

World Health Organization – ICD-11 classification of paraphilic disorders

Kinsey Institute – Historical research on sexual variation and fetishism

Cos’è il fetishism Derivante da “feticcio” (dal portoghese feitiço), il termine Fetichism veniva usato dai mercanti del XVI secolo per descrivere oggetti di culto delle popolazioni africane.
