CBT stands for “Cock and Ball Torture”. It is a BDSM practice involving painful or intense stimulation of the male genitals, with busting, bondage, pressure, percussion, extreme temperatures, or other methods. It falls under impact play and pain play, with a specific anatomical focus.
What it includes
Techniques vary widely in intensity. They range from simple manual pressure or pressure with objects, to rope bondage or leather restraints, up to genital busting and specific devices. The use of ice or heat, carefully, is common in more advanced sessions. Ballbusting is technically a subcategory, but it is often treated separately because it has its own aesthetic and a specific audience.
Safety
Male genital anatomy is not fragile in an absolute sense, but some poorly executed practices can cause serious damage: torsion, hematomas, circulatory problems from bondage that is too tight. The basic rules: never leave bondage on too long without checking, never use strong strikes without warm-up, an active safe word that is respected without exception.
With experienced partners and proper communication, the risk is manageable. Without them, it isn't.
The psychology of pain
For those who practice CBT, pain is not a side effect to be tolerated: it is part of the pleasure. The mechanism is similar to other forms of pain play: the body responds to intense pain with endorphins, and in a context of sexual arousal this response blends with arousal. Many practitioners describe an altered state during intense sessions, similar to BDSM “subspace”. The psychological component—vulnerability, total exposure, trust in the partner—is often described as just as central as the physical sensation.
